# Medical Collections After the FICO Model Change

The score on a free app is often not the score on the tri-merge.

Medical collections are treated differently than they were a few years ago — on _credit reports_ and on some _consumer_ scores. A mortgage file still often uses older FICO models on a tri-merge. This page is a dated snapshot of that gap. It does not say your score will go up. Snapshot as of 2026-08.

This page does not say medical collections “don’t matter,” that paying them will raise your score, or that a consumer-app score is the mortgage score. Dated snapshot only.

## What the bureaus said they would stop reporting

Equifax, Experian, and TransUnion stated they would stop reporting paid medical collections (from July 2022) and unpaid medical collections under $500 (from 2023), with a longer waiting period before unpaid medical collections appear. Those are bureau reporting policies, not a federal guarantee that every medical bill is invisible.

A proposed federal rule to strip medical debt from credit reports more broadly was not a durable, in-force replacement for those bureau policies as of this snapshot. Confirm current reporting on your actual tri-merge — not a 2023 headline.

## What mortgage scores still are

Many mortgage tri-merge files still use classic FICO models (commonly Score 2, 4, and 5 by bureau) rather than the consumer FICO 8/9/10 app score. Newer models that ignore paid collections or de-emphasize medical collections are not automatically the score on your Loan Estimate. A GSE transition to other models has been discussed; confirm what your lender actually pulls.

Paid medical collectionsNewer FICO models and VantageScore often ignore paid collections. FICO 8 still can count collections that remain on the report.Classic mortgage models can still treat a collection that remains on the tri-merge. Paying it is not a promised score increase on those models.

Unpaid medical under $500The three bureaus stated they would stop including these on credit reports from 2023. If it is not on the report, it cannot score.Same report feed — if the bureau no longer lists it, the tri-merge usually will not either. Confirm the actual report. This is not a score-raise promise.

Must it be paid at closing?A consumer score does not answer payoff conditions.Whether a remaining medical collection must be paid at closing is an investor and AUS question, separate from the score. Fannie Mae, Freddie Mac, FHA, and VA publish collection-payoff rules that can treat medical collections differently from other charge-offs. A paid collection that still sits on a classic mortgage score is not a promise that paying it will raise that score.

## Payoff vs score

An underwriter’s payoff condition and a score model are different machines. Paying a collection to satisfy a condition can still be the right file move even if classic FICO does not reward the payment the way FICO 9 would. That is not advice to pay or not pay a collector.

## What happens next

- Do not treat a Credit Karma or issuer FICO 8 as the mortgage middle score.
